Types of Plan
- Basic Plan (Basic) : 1X Policy limit paid at death.
- Terminal Illness (TI) : Half of policy limit paid when diagnosed with terminal illness and 12 months or less to live. The other half of policy limit paid at death.
- Double Indemnity (AD) : 2X policy limit paid when death is result of covered accidental injury. 1X policy limit paid at death due to illness.

Underwriting Requirements
- Complete an IC+ International Term Life Insurance Application, may be completed on-line
- If the total amount of Life Insurance applied for (all companies) is greater than $350,000, a Financial Questionnaire is required. There are two Financial Questionnaires; one is required when the purpose of purchasing the insurance is Personal Protection, Inheritance Tax or in connection with a Private Residential Loan; the other is required when the purpose of purchasing the insurance is Keyman, or in connection with a Commercial Loan. Financial Questionnaires will be provided upon receipt of your application.
- A Medical Questionnaire, completed within 30 days of Application, by a licensed Physician, is required at the following amounts of insurance: ( A Medical Questionnaire will be provided upon receipt of your application)

How Do I File A Claim?
Filing a claim is easy. When there is a claim, you can either:
- Access Client Zone by logging in at https://zone.mnui.com/clientzone/ with your policy number and Date of Birth (or password) to download the necessary forms; or
- Contact MNU Client Relations by e-mail via insurance@mnui.com or by phoning 800-605-2282 or 317-262-2132 (collect calls accepted).
